COVID-19 Workplace Exposure Notification Checklists

These checklists will assist employers in complying with various COVID-19 workplace notification requirements passed in 2020 — AB 685 and SB 1159 as well as temporary California Division of Occupational Safety and Health (Cal/OSHA) emergency regulations.

This form was updated based on both 2023 legislative updates and the two-year COVID-19 Prevention standard adopted by the Occupational Safety and Health Standards Board on December 15, 2022, effective February 3, 2023 after approval by the Office of Administrative Law.
