Do I have to record a violent incident on my Log 300 form?

Many categories of employers are required to record on their Log 300 all fatalities, illnesses and specified injuries that occur as a result of workplace violence. Also, employers must keep a record of any workplace violence committed against a community health care worker and file a copy with the state's Office of Policy, Research and Legislation.

Additionally, employers must create a standalone violent incident log under California’s general industry workplace violence prevention law.
