Pre-Adverse Action Disclosure - FCRA and ICRAA
Use this form to notify an applicant that adverse employment action may be taken against him or her, based at least in part on the results of a consumer report. This notice also must include a copy of the report and a Summary of Your Rights Under the Fair Credit Reporting Act from the Federal Trade Commission. If the decision is based in whole or in part on criminal history, you must also send a Notice of Preliminary Decision to Withdraw Employment Offer.
